CMG is a group of skilled and dedicated physicians who are delivering high quality care, committed to preserving the independent practice of medicine for our physicians and patients. Our physicians practice throughout the Greater New Haven, Shoreline and Fairfield County areas with practices located in 33 cities and towns. We currently have over 400 Primary Care Physician members. CMG is governed by a Board of Directors made up of member physicians from our community based independent practices. We have an executive team led by our President, Dr. Joseph Quaranta, along with our Chief Operating Officer, Director, Care Management, Director, Provider Relations, Director, Clinical Quality and General Counsel. Our team oversees our care management/patient navigators, clinical quality, clinical performance metrics, finance, communications and administrative team members. Our headquarters are located in downtown New Haven.
CMG is dedicated to improving the quality of care that we deliver to our patients using evidence based best practice standards applied through our Clinical Integration programs. We are also committed to providing support to independent community based practices and helping them thrive and prosper. We understand the challenges physicians face and CMG is prepared to help. CMG offers many advantages to our physician members. We have fee for service contracts with several insurers including Aetna, Connecticare, Harvard Pilgrim, United Healthcare, Healthy CT and Multiplan.
Our Clinical integration programs have been in place for 7 years, with active programs in Diabetes, Heart/Stroke, Pediatric Asthma, Behavioral Health, Adult Medicine, Radiology, Surgery and Care Transitions. Each of these programs is led by a committee made up of our physician members who design and implement these programs with the goal of improving the quality and value of care we deliver to the patients we serve. CMG has been very active in developing programs that meet the challenges brought by healthcare reform.
CMG currently has five Accountable Care Organization agreements in place with four commercial insurers, Aetna, Anthem, Cigna and Connecticare, while also participating as an ACO in the Medicare Shared Savings Program. There are over 175,000 patients cared for under these agreements and the number is growing. We are in the process of developing other commercial and Medicare ACO products. The goal is to have all of our patients covered under value based reimbursement models. We have also built a central infrastructure to support all our activities and insure that we will be successful in these efforts.
